    Jewish Family Services of Kansas City Is seeking a Older Adult Care Manager, focused on serving the greater community.


     Jewish Family Services (JFS) supports and strengthens lives throughout Greater Kansas City. We provide essential human services for people facing challenges in everyday life or times of crisis — regardless of faith, age, culture or lifestyle. Their mission provides high quality counseling, older adult services, education and life crisis assistance while serving as a comprehensive resource for the entire Kansas City community. The organization has enjoyed outstanding operational and financial success over the years. 


    Reporting directly to the Director of Older Adult Services, the Older Adult Care Manager will work to empower older adults to age in community through an array of supportive services including care management, transportation and home maintenance. This care management role is based in our Brookside office and provides services to clients residing in their own homes in the community.



    Performance of the following essential duties to be done within the context of National Association Social Work’s core values: service, social justice, dignity and worth of the individual, importance and centrality of human relationships, integrity and competence:

    • Represent JFS and collaborate with the community partners on various aging services and programming
    • Provide community-based care management to older adults and/or their caregivers.
    • Craft a client-centered goal and related strategy, appropriate to the needs of the person served.
    • Implement client-centered, trauma-informed interventions with clients.
    • Document all client contact in accordance with agency procedure and policy.
    • Coordinate and partner with health care providers, caregivers, family members and other community programs and resources.
    • Provide information and referral.


    Education, Knowledge, Experience:

    • Bachelor’s or Master’s Degree in Social Work or other relevant discipline.
    • Experience providing client-centered, trauma-informed care management services including assessment, goal planning, intervention and resource connection.
