For more than 30 years, reStart Inc. has provided housing and services to homeless youth, families, men, and women to end homelessness in the Kansas City community.

 

A pioneer in the field, reStart is the only homeless agency in Kansas City serving all homeless populations, including singles, couples, gay, straight and transgender individuals, as well as youth ages 12-18, non-traditional families, persons with HIV/AIDS and individuals with substance use and mental health related issues.

 

They are seeking a Healthcare for Homeless Veterans (HCHV) Care Monitor to join their team. This position is part-time.

 

Responsibilities

The HCHV Care Monitor will assist HCHV case managers and clients with coordination of appointments and housing searches.  Care Monitor will promote a safe drug free environment.  HCHV Care Monitor will assist clients with life skills and needs around the shelter.  HCHV Care Monitor will customize services to each individual's need and will prioritize services that promote housing stability.

  • Drive clients to appointments and assist onsite, as needed.
  • Coordinate client schedules.
  • Monitor premises to ensure a safe and drug free environment.
  • Assist with maintenance emergencies, as needed.
  • Utilize trauma responsive techniques to deescalate crisis or potential crisis situations.
  • Assist programming staffing as needed.
  • Document all significant events/encounters.        
  • Network with community agencies that provide services to veterans.
  • Attend required meetings (community, all-staff, etc.)

Qualifications

  • Excellent interpersonal communications skills                
  • Knowledge of community resources
  • Computer skills – Microsoft Office, database entry                     
  • Must have a valid driver’s license                      
  • Must pass a criminal records check and pre-employment drug screening
  • Ability to provide trauma responsive services 
  • Experience in social work or human services field preferred; experience may be substituted for degree; veteran preferred
  • 1 year of relevant experience working with homeless individuals, veterans and/or other underserved populations.
